--Low-temp dishmachine observed with buildup of scale, filth, and food residue, and not cleaned before use and frequently throughout the day.

--Accumulated soil, food residue, pooled condensate, and debris in cracked / exposed areas of degraded floor cover (noted below). Box fan in kitchen area in need of a detailed cleaning to remove the thick build-up of dust from the blades and blade guards.
- Cleanability of Floors, Walls, & Ceilings (46.921)
--Floor covering in disrepair, floor tiles broken, missing, sub-floor exposed throughout the kitchen / food prep, warewashing, and bar areas. One (1) floor drain cover missing, next to the chest freezers. Cleaning is an issue, as noted above. Degraded, peeling / flaking paint on wall in equipment / stockroom storage area.
- Light Bulb Protective Shielding (46.922(a))
--Eighteen (18) fluorescent tube lights located in the kitchen and warewashing areas not shielded or shatter-resistant, as required. Noted on previous IR.
