I need help with an identification. I was in a wooded part of a wildlife refuge in Delaware and happened to see this leaf. I'm more interested in what's on the leaf than the leaf itself. No other leaf around this one had these spots on it (or even one spot) even though there were more of this leaf type there.

Is it something living? Is it something common? If it is some type of disease, why only one leaf with it? I don't have a garden and know very little about plants/trees/weeds. Can anyone help? Thanks in advance.

The fact that its only on one leaf out of many suggests something intentional. Eggs deposited by some insect? Send the image to the wildlife refuge office. Maybe they know?

Right!
These colorful spots are caused by the ocellate gall midge, Acericecis ocellaris, in the family Cecidomyiidae. Although rarely seen, adult ocellate gall midges are mosquito-like flies and resemble other midge pests like the boxwood leaf miner.
This fly deposits eggs exclusively on the underside of red maple leaves. This causes a chemical response in the leaf resulting in gall formation.
